Insurance co-payments are due at the time of the visit.
Bring Your Insurance Card
Since we are required by insurance companies to verify your insurance, please bring your insurance card with you so that we may make a copy for our files. If you change insurance companies or your insurer provides you with an updated card, please bring it to the appointment.
